Usually not the access points. The common causes are too few APs placed by floor plan rather than by survey, channel overlap with neighbouring networks, an uplink bottleneck behind the AP, or all traffic sharing one flat VLAN. A survey identifies which of these applies before anything is replaced.
If you want VLANs, quality of service, port security or any visibility into what the network is doing, yes. Unmanaged switches are adequate only for the smallest flat networks.
Usually. We test what is there and report which links are performing to specification. Replacing an entire cable plant is rarely necessary — replacing the worst 10% often is.
